ASCO 2026｜陆明教授：epNEC 前瞻性队列数据深度解析，DLL3高阳性率及高表达率夯实靶点地位

前言

肺外神经内分泌癌（epNEC）异质性显著、分化程度低、侵袭性强，患者整体预后较差¹ 。当前epNEC临床治疗仍存在明显短板，现有一线标准治疗（SoC，含铂双药化疗）的神经内分泌癌（NEC）患者结局较差，二线领域尚无获批的标准治疗方案，前瞻性化疗临床研究显示，此类患者ORR仅维持在10%~25%，远期生存仍亟待突破^2,3 。

DLL3在epNEC等NEC肿瘤细胞表面呈特异性高表达，而在正常组织中几乎不表达，为epNEC精准靶向治疗开辟了全新突破口³ 。Obrixtamig作为一款新型DLL3/CD3 IgG样T细胞衔接器（TcE），其I期研究结果显示，Obrixtamig单药及联合治疗方案分别在epNEC后线及一线治疗中展现出可观的持久抗肿瘤活性，且耐受性可控，相关研究成果已先后亮相2025年美国临床肿瘤学会（ASCO）、2026年美国临床肿瘤学会泌尿生殖系统肿瘤研讨会（ASCO GU）、2026年欧洲神经内分泌肿瘤学会年会（ENETS）等国际重磅肿瘤盛会。2026 ASCO大会进一步披露了I期研究中筛选和治疗的晚期epNEC患者肿瘤DLL3表达水平前瞻性数据^3-6 。值此之际，特邀 北京大学肿瘤医院陆明教授 深度解读DLL3在epNEC患者中的表达模式，分析Obrixtamig治疗epNEC的研究结果及临床潜力。
