头颈部鳞癌是指原发于口腔、鼻腔鼻窦及咽喉部粘膜鳞状上皮的恶性肿瘤,其发病率居所有恶性肿瘤的第7位[1]。头颈部鳞癌的发生主要与吸烟、酗酒,以及HPV病毒感染有关[2]。目前,临床上将原发肿块较大(T3期以上)或淋巴结转移(N1以上),即临床分期为III-IVb期的患者定义为局部晚期患者。这一类患者异质性强,治疗模式多,在治疗的过程中,既要关注患者的生存时间,又要尽可能地保证患者的生活质量,因此,最能体现多学科团队诊疗(MDT)模式的优势,也是目前临床诊疗的难点和临床研究的热点。

目前,对于局部晚期头颈鳞癌的患者,国内国际指南均一致推荐两种主要的治疗模式。对于可手术切除的患者,主要采用手术切除,再根据术后病理分期行辅助放疗/放化疗的模式,而对于不可手术切除的患者,则主要采用以同步放化疗为基础的治疗模式。然而,对于接受手术治疗的患者,由于手术切除的范围通常较大,对患者吞咽、发音、呼吸等功能将造成显著影响,严重降低了患者的生活质量。并且,即使经过根治性手术和术后辅助治疗,仍有接近60%的患者出现复发和转移,总体五年生存率仅为45%左右[3]。而接受同步放化疗的患者,虽然其器官保全率较手术患者高,但由于肿块较大导致的乏氧等多种原因,其疗效并不理想,总体五年生存率仅为33.6%[4]。因此,探索在保证疗效的同时,尽可能提高器官保全率,提高患者生活质量的治疗方法,成为头颈肿瘤医生关注的重点。

诱导治疗是指在手术、放疗等治疗之前进行的全身治疗。诱导治疗具有通过缩小肿瘤实现器官保全[5],通过消灭微小转移病灶降低复发转移风险[6],通过在体评估肿瘤的治疗的反应性指导后续治疗方案的选择等优势。随着药物治疗水平的进步,诱导治疗也从诱导化疗进入了靶向和免疫治疗的时代,给局部晚期患者的治疗带来了新的选择。

1.  化疗在诱导治疗中的作用

TAX323/324研究是最早的关于局部晚期头颈部鳞癌患者诱导化疗的III期临床研究。TAX323研究共纳入了358例不可切除的局部晚期头颈部鳞癌患者,接受DPF方案或PF方案诱导化疗及后续的同步放化疗,主要研究终点为PFS。研究结果显示,TPF相较于PF方案,显著延长了患者的PFS和OS[7]。TAX324研究则入组了501例不可手术或因器官保留未行手术治疗的局部晚期头颈鳞癌患者,分别接受DPF或PF方案诱导化疗,以及后续的根治性同步放化疗。与TAX323研究的结果类似,TAX324研究的结果显示,无论在不可手术切除亚组还是可手术切除亚组,DPF方案诱导化疗均较PF方案的诱导化疗显著提高了患者的PFS和OS[8]。TAX323/324研究的发表奠定了DPF方案在局部晚期头颈部鳞癌患者诱导治疗中的地位。虽然DPF方案诱导化疗取得了一定的疗效,但目前在临床应用过程中仍存在争议。首先,这两项研究均是对比的不同诱导化疗方案间的疗效差异,并非诱导化疗联合同步放化疗与同步放化疗的比较。而TTCC 2002[9],DeCIDE[10],PARADIGM[11],以及RTOG9111[12]等研究的结果显示,诱导化疗联合同步放化疗相较于单纯同步放化疗,并不能改善患者的总生存。因此,目前对于同步放化疗基础上的诱导化疗能否改善总生存,尚存在一定争议。此外,DPF方案治疗的患者具有更高的血液学和消化道不良反应[7, 13],这也限制了该方案的临床使用。因此,虽然诱导化疗取得了一定的临床疗效,但其总体疗效提升较小,毒副作用较大,仍需探索更加高效、低毒的诱导治疗方案。

2. 靶向治疗在诱导治疗中的作用

头颈部鳞癌普遍存在EGFR的高表达,以EGFR为靶点的靶向治疗在头颈部鳞癌的治疗中发挥重要作用。EXTREME研究[14]的发表奠定了西妥昔单抗在晚期头颈部鳞癌一线治疗中的地位,该研究发现西妥昔单抗联合化疗在显著提高疗效的同时,并不增加严重不良反应的发生率。靶向治疗因其高效和相对低毒的特点,也被广泛地引入局部晚期患者的治疗中。目前西妥昔单抗在局部晚期头颈部鳞癌患者中的临床研究的主要思路是使用西妥昔单抗替代部分化疗药物,从而在保证疗效的同时,达到降低不良反应,提高耐受性的目的。目前,共有6项临床研究报道了西妥昔单抗在局部晚期头颈部鳞癌患者治疗中的疗效和安全行,总结如下表所示:

3. 免疫治疗在诱导治疗中的作用

以PD-1/PD-L1单抗为代表的免疫检查点抑制剂的上市改变了恶性肿瘤治疗的格局,免疫治疗是目前各大肿瘤中的研究热点。在头颈部鳞癌的治疗中,Keynote048研究奠定了帕博利珠单抗在晚期头颈部鳞癌患者中的一线治疗地位[21]。目前,免疫治疗也在由晚期一线快速向局部晚期患者的治疗推进。免疫治疗在局部晚期头颈部鳞癌患者治疗中的参与模式主要包括围术期免疫治疗,免疫+同步放化疗等。目前免疫治疗在局部晚期头颈鳞癌患者中的临床研究总结如下表所示:

屏幕截图 2022-07-30 192242.png在可手术切除的局部晚期头颈鳞癌的治疗中,PD-1单药作为新辅助和辅助治疗是目前的主流模式,从现有临床研究结果看,免疫单药治疗具有良好的安全性,但其有效率在不同研究中报道的差异较大,且目前的临床研究主要以近期疗效作为主要研究终点,近期的病理缓解能否转化成生存获益仍有待进一步的研究证实。而在不可手术切除的患者中,免疫治疗的研究贯穿了诱导、同步和辅助治疗阶段。在前期帕博利珠单抗的IB期研究中,在同步放化疗基础上联合帕博利珠单抗作为同步和辅助治疗,表现出良好的安全性。然而,该领域中最大的打击来自于JAVELIN Head and Neck 100研究的失败。这项研究是迄今为止样本量最大的III期临床研究,该研究中,阿维鲁单抗的治疗贯穿了诱导、同步和免疫阶段。然而研究结果发现,在同步放化疗基础上加用阿维鲁单抗并不能改善患者的无进展生存时间。该研究失败的原因可能是同步放化疗期间的急性毒性导致患者营养状态的恶化引起的免疫功能下降,以及处理急性毒性所使用的激素、抗生素等药物影响免疫治疗疗效等原因。此外,该研究同时提示,可能需要进一步探索疗效预测的生物标志物,以精准地筛选出可能从免疫治疗中获益的人群。虽然JAVELIN Head and Neck 100研究失败了,但在局部晚期患者中使用免疫治疗的探索并未停止。目前,Keynote 412,REACH等临床研究都正在进行中,相信未来会为局部晚期患者使用免疫治疗提供更多的证据。

4. 总结与展望

局部晚期头颈部鳞癌是目前头颈部鳞癌患者治疗中的难点。以手术为基础的治疗可取得相对较好的疗效,但对患者的生活质量影响较大。以同步放化疗为基础的治疗有助于器官保全,但其总体疗效并不十分理想。诱导治疗为实现在保证疗效的同时,提高患者生活质量提供了新的思路。目前,化疗仍然是诱导治疗的基石。靶向治疗和免疫治疗也在诱导治疗中展现出良好的前景,但目前尚缺乏大规模随机对照研究以证实其疗效和安全性。未来,我们需要进一步探索化疗、靶向、免疫治疗的最佳结合模式,并寻找疗效预测生物标志物,以精准选择获益人群,使患者实现生存时间和生活质量的双赢。
